Position: Licensed Vocational Nurse (LVN) – Private Duty

Pay Rate: $35.00 – $50.00 / hour (Based on experience)

Shift: 12-Hour NOC (Night) Shift

Location: La Jolla, San Diego North County

About the Role

As a Private Duty LVN with Cheer Health Care, you will deliver high-quality, one-on-one skilled nursing care to seniors and adults with disabilities. We are looking for compassionate nurses with vibrant personalities who want to make a meaningful, daily difference in our clients' lives.

Key Responsibilities

• Skilled Clinical Care: Provide exceptional hands-on care, including wound care, medication management, and the ongoing monitoring of chronic conditions.
• Care Coordination: Collaborate closely with physicians, rehabilitation services, and other healthcare providers to ensure seamless, comprehensive care.
• Client & Family Education: Educate clients and their families on diagnoses, treatment plans, and wellness strategies to ensure clear understanding and peace of mind.
• Meticulous Documentation: Maintain highly organized, accurate, and timely clinical documentation of all care provided.

Requirements

• Current, unrestricted California LVN license
• Graduation from accredited practical or vocational nursing program
• 1 year of experience as a Licensed Vocational Nurse
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening Documentation within the last year
• Current BLS certification
• Valid California drivers License and insurance
• Cell phone with internet access
