Shawnee Health is seeking a Care Coordinator Supervisor for Shawnee Health Alliance. This position has the opportunity to be a remote position. This position reports directly to the Community Services Unit Director. Responsibilities

• Supervision of care coordinators, caseload management
• Initial and continued training of care coordinators including home visits
• Review of care plans and client files
• Development of community resources, and establishment of in-service training programs
• Understanding and adherence of IDOA rules and regulations, EAAA programs through the Older American's Act as well as recognizing staff qualities for upward mobility.

Requirements

• Bachelors degree in human service field and/or RN
• Preferred MS degree in social services with supervision
• 2+ year's experience with same population

Starting salary - $51,320 Full-time employee benefits include:

• 12 vacation days earned in the first year
• 11 paid personal days/holidays per calendar year
